Create a diagram showing the different moon phases and their names with this cut-and-paste activity sheet.

Phases of the Moon Worksheet

With this activity, students will cut out each of the moon phase pictures and labels. Students then can glue each of the phases in the correct order on a separate piece of paper while putting the labels underneath each picture. It may also be helpful to remind your students to draw the sun on their diagram as well!

This download includes a version for both the northern and southern hemispheres.

An answer key is included with your download to make grading fast and easy!

Tips for Differentiation + Scaffolding

If there are students who need an additional challenge, consider giving the moon phases activity of the opposite hemisphere they live in to complete.

For students who need additional support, invite them to reference one of our moon phase posters to help them with this activity.

Easily Download & Print

Use the dropdown icon on the Download button to choose between the PDF or editable Google Slides version of this resource.

Because this resource includes an answer sheet, we recommend you print one copy of the entire file. Then, make photocopies of the blank worksheet for students to complete.

Additionally, project the worksheet onto a screen and work through it as a class by having students draw the phases of the moon in their notebooks and label each picture.
