Support students in reaching a personal or academic goal by creating a customized achievement plan.

This student goal-setting worksheet is best used as independent practice for your social-emotional learning lesson. It can also be completed with a guided small group or as a whole class activity by projecting the worksheet via a smartboard.

Student Goal Setting Worksheet Scaffolding and Extension Tips

Challenge students to either fill out a plan for both a personal and an academic goal, 2 personal or 2 academic goals, by a certain date. They could also create art (like a magazine collage) to support goal setting and present the plan to their class.

Students who are struggling to understand this concept might verbally explain to the teacher the goal they want to set and the date by which they want to achieve the goal while the teacher assists with writing complete sentences for this resource.
